Independent Business Status
Thirdeye Studio, its artists, contractors, and staff remain independent from any host, venue, brand, or organizer.
Venue & Safety Requirements
The Host or Venue agrees to provide a clean, safe, and compliant environment. Thirdeye Studio reserves the right to refuse services if conditions are unsafe, unsanitary, or non‑compliant.
Client Documentation
All clients must complete required Studio intake forms and service‑specific waivers prior to receiving any Services.
Assumption of Risk & Liability Release
The Host acknowledges that tattooing, ear piercing, and permanent jewelry services involve inherent risks including but not limited to infection, allergic reactions, scarring, dissatisfaction with results, or healing complications.
The Host agrees to release, defend, indemnify, and hold harmless Thirdeye Beauty & Wellness, LLC, its owners, artists, contractors, and staff from any and all claims, damages, injuries, losses, or legal actions arising from or related to:
Off‑site services
Venue conditions
Event guest actions
Alcohol or substance use
Third‑party involvement
Insurance
Thirdeye Studio maintains professional and general liability insurance. The Host is responsible for any additional coverage required by their venue or event.
Off-Site Pop-Up Flat Fee
A non-refundable flat fee of $300 is required to secure any off-site pop-up or private event booking. This fee covers scheduling, staffing, preparation, and administrative costs. The fee is non-refundable under all circumstances unless otherwise agreed to in writing by Thirdeye Studio.
The flat fee does not apply toward service pricing unless explicitly stated in writing.
Media Release
The Host grants Thirdeye Studio permission to capture and use photos or videos from off‑site services for marketing and promotional purposes unless revoked in writing.
Governing Law
This Agreement shall be governed by the laws of the State of Florida.
Entire Agreement
This document represents the full agreement regarding off‑site services and may only be modified in writing.
